A simple Survey that I can't seem to get my head around

I am wanting to do a simple questions survey.

 

I have a participant table, a questions table, and a Results table.

 

I would like to show all the questions entered  from the questions table in a portal linked from the participant.

 

I would like to record the result of the question in the Results Table.

 

I understand that you can create a copy of all the questions in the results table for each participant, but I don't want to fill the results table up with unnecessary records.

 

The attached file is a simple version of what i am trying to do.

When I enter an answer in your startup layout all four questions then have the same answer. This is likely because of the Cartesian relationship.
