Need help formatting print for special layouts

I know many people find paper directories (eg Filofax) archaic - but I'm trying to figure out how to format a layout to print to specialized paper, ultimately destined for filofax type notebooks.

In the 1st case I'm working on, it's a simple half-page, and I suspect I can manage that one through the printer driver by printing 2-up.

The harder case I'm asking for ideas on has tiny filofax-sized paper, 4 per 8.5x11 sheet. The individual pages are 8.1cm x 12 cm, in portrait mode arranged 2x2 on the paper. I'd like to print an address directory - alpha by last name, print name, phone, address, email.... in a list.

Your ideas and suggestions are welcome!
