Bad password

We have had a password go bad; the passwords work for other users, but for some reason, we cannot get one password to work. It hasn't been changed by the end user, Has anyone ever seen anything like this? Could this be somewhere in a script (you can tell this isn't my database.) Any direction would be appreciated.
