Simple Portal Question

Newbie here trying to set-up what I think should be a simple portal. I want a button that will allow me to add the current date in one field and then on the adjacent line it will allow me to enter as many lines of free text as I want. Each time I press the button the most recent date and note will be added to the top of the list. I can draw this out (see illustration below) but can't seem to define the fields, relationships and write the scripts to get it all to work.

It seems to me I'm making it harder than it actually is. Any help is greatly appreciated.

Each time you click on the button you create a new portal row. What you have to do is make the portal row many lines long. Make the notes field the same number of lines and put a scroll bar on the notes field.
