Condtional value list with Lookup data

I wonder if someone can give me some hint on solving the following problem:

For simplicity, I use the following simple scheme. I have an item table with several categories of products, product names of each category, product ID and cost (see Item layout); and a customer layout where I use the conditional value list technique to isolate one category of related items and then choose the correct item from the isolated category. I use the Lookup field option for product ID and cost. Somehow, each time I choose the proper item in a category, only the product ID and cost for the first item of the isolated category is shown. The product ID & cost fields would not show the proper values if I choose 2nd or 3rd item of the isolated category. The bottom part of the customer layout shows the items in the isolated category (see the enclosed zip file)
