I have a table for patients follow up for chronic condition, that include Patient_ID, visit_date and other fields that monitor the patients’ status.

I would like to have a script that can find for me the number of patients who did come 6 times to the outpatient for their follow up visits in the last 6 month (every patient has 1 visit/month) and export that value to an excel sheet.
