Related field outside of portal not working right

I have a portal that displays doctor's names and address that I choose from a value list. The portal is in the Client File. The doctor's names and addresses are in the Medical Provider File. The portal and the value list get the information from the Line Items File. This all works great. My problem is that I need to have a field outside of the portal where I can choose one of the doctors as the Treating Doctor and that field will contain the doctors name and address spaced apart appropriately so that I can use that field for a government form on another layout. I also need another field outside the portal which will display the doctor's name and address in a form for a letter to the doctor, and I also need to choose from the same doctors. Like I said, the portal works great. But which ever doctor I put on the first line of the portal automatically goes into the treating doctor field. I want to be able to pick a doctor for that field. I assigned the same value list to this field but it gives me an error message saying that this field is not modifiable whenever I try to choose a different doctor. I did define this field as a calculation field so that I could add in the appropriate extra space in between name and address fields so that it would correctly fill in the form. Am I going about it all wrong? The other field is doing the same exact thing. Originally I was not even going to create this field for the letter to the doctor because I wanted to create a script that would loop through each doctor in the portal and print the letter to each doctor in the portal. That was the ideal situation, but I can't figure that one out at all. It kept only putting the first doctor in the letter and looping through and putting the first doctor in again. So I decided that I was going to have to choose one doctor at a time to put in the letter layout, but now I can't even do that because it still won't let me get past the first doctor. Please help!

Because the treating doctor is associated with the specific line item in the portal, If you want to show the treating doctor, it should be within the portal line item. If the treating doctor is different for each line item (exam or office visit) then only the 1st treating doctor will display in the field ouside the portal.

Why not just show the treating doc in the portal. Then you will be able to glance at who the patient has seen recently especially if sorted in descending date order.
