Page Header Does Not Print in FMP 5.0 & 5.5

I have a problem with the page header is not printing in Windows 2000 on a Brother 1240 Laser printer. When I fax the form using Faxport, it print the header. I had this problem with FMP 5.0 & 5.5

Notes:

I am using the only print driver there is for Windows 2000.

It does print with Windows ME & Brother 1240 Laser Printer.
