Layout options for using buttons to insert (append) text into Notes box

I want to record horse treatment notes in a patient's Visit Notes layout. To aid in entry speed, I want to place a series of buttons to the side of the layout (this will be for iPad), that when tapped, will add(insert, append) to text in a Notes field. Initial thoughts are to design the layout with the buttons, and associate a script that simply inserts the text into the Notes field, for that patient, on that visit. Is this too simple, or should I be setting up a more complex structure?

OK, so I had a go anyway, and did it as I described above, and is working as expected.

 

Decided that I would investigate the option of 'tabbing' the Visit Notes layout to break it into sections. 

 

I basically copy/pasted the bunch of buttons I created, and also added the notes field, onto  of the tabs.

 

This time in the tabbed layout, when I click on the buttons, I am unable to see any inserted text, subsequent to the first button/insertion operation, unless I navigate out of that layout and back again. i.e. the Notes field doesn't seem to be refreshing itself instantly as I click on buttons to insert text here. The only text that is inserted, it that scripted by the first button only.

 

Obviously I need to do something different, for a reason i don't know yet. Any thoughts?
