Two portals one to enter data and the other to view all the data

Good morning all,

I am trying to insert two portals in a form but, I have followed other samples but for some reason I cant get it to work in my database, I am attaching a sample of what I am doing hopping a guru can tell me what I am doing wrong.

 

In short I have a ticketing system and I want to be able to enter ticket status on one portal but at the same time be able to view the status history in a portal below.
