FMP 16 - Migrated database lands in wrong fields

Newbie here.  I'm trying to transfer an address book database from FM 11 to FMP 16, using  .fmp12 suffix on the files.

When I open FMP 16 I see the data, but the fields don't match up.  e.g. Street Address lands in the First Name field. 

Any advice appreciated.

Thanks for the response bcooney.  Yes, I had done the file conversion without success.

But since my OP I was able to go back into my FP11 files and reconfigure the layout, so that the data landed correctly when converting.
