How do I print ONLY items checked items?

Hi all,

I have created a detailed database for calculating bids for my video company... I would like to create a questionnaire that I fill out, placing a check mark next to each item that applies to the bid, then I want ONLY those items that I checked to appear when printing the bid for the client....

Any thoughts about how to do this?
