Problem with images in a container

I have about 650 images in my db. The first 648 images worked fine. However, now when I insert an image into my container field, the image appears very small in the container rather than filling it like the first 648 times. I created a new db as a test and had no problem inserting and displaying the same image. PDF file is attached for reference. Any idea how to fix this?

Edit: this problem occurs with all images I try to add to the db, it is not specific to just one file.

What happens if you insert the 648th image into a clone ( empty records ) of your first database?
