Simplifying data entry using values or buttons

My knowledge in Filemaker Pro is limited to say the least, but I am aware it is very powerful. I am working with a hand surgeon who has a very basic request.

He wants to be able to click a button (lets says its labeled Elbow Pain) and when he clicks it, a pre-configured string of text is entered into a text output field. So, when he clicks the Elbow Pain button, the string of text "The patient experiences elbow pain" is inserted into the field.

He wants to have a number of different buttons that have this function so that he can ask a patient typical questions, and instead of typing and answer, just simply click a button. So he may click the following buttons:

Male Patient

Elbow Pain

Occured in Sports accident

Needs pain medication

and after clicking these buttons, he gets the following results in a text document:

"The patient is a male. The patient is experiencing elbow pain. The patient first experienced pain in a sports activity. The patient should be prescribed pain medication."

What would be the best way to do this? Someone mentioned maybe not using buttons and using value lists with pop-up menus would be better? Can someone point me in the right direction? Thanks.
