Thanks for your inquiry, you're trying to help me - and I'm rude and don't respond - Let me apologize. Life has just been beyond hectic - and it makes it all the more crucial that I get this resolved.

I've attached a Zip file with files within it: a simplified FMP example - and a PDF that shows a sketch of the intended output for each patient.

Because my life is also currently hectic, I didn't have a lot of time to devote to this. I'm sure someone else can clean up this solution a bit, but at least it seems to work well enough for now. I didn't change any of your existing relationships, only added a few, along with value lists, to make this work with its existing setup. I didn't have time to comment it either, so if you're completely baffled, let me know and I'll try to explain it.
