Strange problem is occuring, where the whole record is sometimes duped when users are creating new record. This is occuring aproximately once a month.

Mind you, they DO NOT go to the menu, there is a button specified to create new record/request. So no, they don't click on "Duplicate Record" menu by mistake.

This database has Auto-generated unique PK field, on which it connects to other databases. But it also has a cancatination calculated field, on which it connects to other databases (this is just a nature of this solution).

Has anyone ever experienced anything like that?

I'll bet is one of the "other" fancy functions pulling-in data from another file.

Seems to me that there's something in either the calc fields or the relationships that can cause the info to appear to be duplicated. Check to see exactly what info is in the "original" record that is "duplicated", and cross-reference it with the calcs & relationships, and you'll probably have your answer.
