Need to print related records from multiple tables

I'm trying to replace a hand written form system. The main form currently has spaces for a fixed number of related type data such as 3 people, 2 geographic locations and 3 lines for notes.

It needs to have dynamic space for multiple related records such as 0 or more people (name, address, phone, etc.) and 0 or more locations (building a in area 1, building b in area 1, etc.) and the notes section may be anywhere from 1 line to 10 pages.

The final report may fit on one page at times but when it can't fit, it would be good to have a placeholder that refers the reader to a different page.

So my first problem is getting a dynamic quantity of related records to print within a 'perceived' form body and the second is an idea of how to put a placeholder type note where the data would be if there are too many records.

Also it can't be monospaced and the report will need to include a lot of lines for boxes. There are as many as about 200 fields on a single page. My guess is that these restrictions will make the solution harder to reach.
