Entering multiple goals using the same layout & fields

I have a layout where I want to enter in a persons name and then be able to enter in multiple goals for that person that include the goal, who is responsible for it, and the target date for achieving it.

Right now I have the fields set up as follows:

•MspGoal1 – (text) MSP Goal.

•MspGoal1Staff – (text) Staff member(s) responsible.

•MspGoal1Due – (date) Targeted Achievement date for goal.

•MspGoal2 – (text) MSP Goal.

•MspGoal2Staff – (text) Staff member(s) responsible.

•MspGoal2Due – (date) Targeted Achievement date for goal.

And so on for up to 5 goals. Is there a way where i can have just the MspGoal, MspGoalStaff & MspGoalDue as fields and a person can enter in multiple goals? Would I set up a seperate goals table and use a portal somehow?

Hopefully a picture and a file are worth a thousand words. Username for the file is Admin there is no password. picture can be found at
