I have a membership renewal form that is PDF, is there any way I can import this thing as a layout template, so I can use it as a report for printing and screen editing?

The source of the form is a Word file.

Otherwise it's a long weekend of fooling with layouts.

Hmm, well you might consider scanning it and placing it on your layout. You can add fields on the top. It's worked for me before when filling in forms.
