Trouble using Custom Dialog "Input Fields" with Date

Hi, I created a test DB to try and script a way to pass along a customized date into a new record when it is created, but it always creates the new record with a blank field. Can someone see why this script is resulting in a blank? I've attached the database, I'm using FM 10 for the Mac.
