It would nice to be able to have at least a warning during the install informing you that FMSA is now going to wipe your entire IIS metabase (i.e. all 50+ site configurations on a dev box : )

Agreed. What's worse is that I find that sometimes with installs even with the option of using your existing version of PHP, it tends to still modify settings. :
