Double log in

I am having this weird issue that i have not been able to solve. When I log in to my database I am asked for my name and password as usual. Then once I log in a 2nd box asking me for a log in and password pops up again but this time my username and password do not work. But then once i hit cancel the database will open. This is a very frustrating issue does anyone know what it could be?

Are there any related files for the solution you are opening. If so do you have the same account name and password in each?
