FM PHP Site Wizard - Look Up Fields Not Populating with Data

I've got a simple order form in FM 11 that works fabulously with the client. However, in the PHP site-generated version, the look up fields do not bring any data in. It's a single database containing 5 tables. Can anyone offer some assistance? Thanks.
