Something is wrong with my CDML tags

Ok, I think I've solved the problem of the sumbission by taking all the check boxes out of the form. My syntax is wrong. I used the CDML tool to create compatible check boxes, but the CDML tags are showing up on the html document. Is there some header line that needs to be included to make the CDML extensions recognizable? How do I hide these tags?
