Converting information to bill on a hcfa 1500 form

Perhaps this is not the best are of Fm Forums for this post, but I was wondering if anybody could provide me with information, templates and calcualtions needed for billing on the HCFA-1500 form. It seemed simple enough when launching into this task. I keep coming to dead ends and infromation that is inaccurate. So any info would be appreciated.

Oh you've hit the mother lode!! I'm in the middle of designing a HCFA-1500 as we speak! I have a copy of the form placed on a layout, I lay my fields over the top in the specific spots, then will delete the template from under it and it'll be ready to print directly onto the HCFA1500! It's currently functional (script for 6-line lineitems, etc) but a few fields need realigning yet. If you would like it, let me know and I'll attach an fp5 for you!

I hope you still are subscribed to this forum. I see that this discussion was some time ago. I am trying to overlay my fields on a HCFA image. My clearinghouse then wants a txt file. Do you know how would I print these fields to a txt file?

Also, how come the field is not lined up the same in Browse mode and Print mode? I'm using FMP 8.5 by the way.

Your clearing house is looking for an EDI file to process. This is not the same thing as you are doing. I do not thi k that filling out a form 1500 will do it for you.
