You can use a basic CDML URL with "-new", for example:

http://myaddress/FMPro?-db=mysurvey.fp5&-format=survey.html&ir=xyz&cust=joe blow&phone=12345678&etc=etc&-new

How you use them in the "mysurvey.html" Format file is up to you.

Thanks, Garry. I used your new record URL, then changed "submit=-new" to "submit=-edit" on the survey form. It is now working perfectly.
