how about making a calculation field that combines all those fields into another field, and then make that a unique field?? is that possible??

It's best to look at phone number or some other field to determine uniqueness. For example, is Rob Moore really Robert Moore and/or Bob Moore or Rob More (spelling)? Uniqueness is a very hard thing to implement for names. If a human can't determine if entries are duplicates, a computer program has no chance.

In answer to "bman", you can't verify uniqueness in a calc field. You can only search for duplicates.
