Why is the "New..." button is greyed when I want to create my own toolbar?

I looked at the help file without success.

I have never created a toolbar before and I would like to create my own buttons.

Either it is not available due to a custom menu or access privileges.

Keep in mind that the customization of the toolbar is end user specific, not something a developer can set for everyone.

IOW, a developer can control what actions the end user can perform but not which items are set on the actual toolbar itself.
