So Open with Bookmarks is an option when you Save as PDF.

Is there a way to populate bookmarks from a field in the header?

If you cannot populate the bookmarks, why would you set it to open with bookmarks panel?

Because you might be using the append to PDF to one that has bookmarks that you want to utilise.

Agreed it does seems a bit superfluous as it stands.
