How to See last 5 result in that table's layout?

I want to create a table that has only previous entries of chosen person.

Tried to use portal tool, but there is no way to choose its own page.

 

I want to enter and see results on same page.

The Portal tool offers a number of options in the Setup. Look at the Script step Go To Related Record (GTRR) which offers the method to have a button in the Portal Row with a script that will Go to the Related Record that can show the data in either a New Window, or the Related File.

 

To see the last 5 entries in the Portal, you need a date and sort the portal by the date.
