Portal Example Request

I run a database for the hospital I work at. On the main screen is a portal where I manually add new records by going to the first blank line in the portal and manually entering the details in.

To make it easier for others to use the database, I would like to have a button on the main screen which takes users to a layout where they enter the details. When they hit the Enter button on this screen, they are taken back to the main screen - and the portal contains the details entered sorted so that the latest data is on the top line - there is a date field.

Just requesting a file that I can look at so that I can learn how it is done and apply it to the database I currently use.
