I've placed the field on the layout, but it does not refresh until I click on the layout. I've tried using a script trigger, but again it does not update until I click on the layout. That is the expected behavior: summary fields will not refresh until records are committed. What exactly are you doing that changes the count? You could attach a script trigger to that.
