Good 3. Standard 4. Poor 5. Bad Radio button selection is usually going to give you a better understanding of your users true feelings ... Numbers 0-100 may render unusable results. (its quite abstract and too subjective) If you want to use a graphic representation i think the ITunes 5 star rating method is really clear and intuitive... most people know that 5 stars are the best and 1 is the worst.
