Description of Duties: Add basic functions to standard template

I need to get a quote on adding some basic functions to the standard invoices template in FileMaker pro 12. I need the invoice page to have a bill to address as well as a ship to address. I need to be able to print a packing list as well as the invoice.

Theses are the 2 crucial things I need added to the system. Afterwards there will be other customizations needed that would be billed separately on a per job basis.
