I'm using FM 12, Adv. Is there a way FM can create a simple web form that users can enter data and then return to me?

You posted this in 'FileMaker WebDirect'. In order to use WebDirect, you need FileMaker Server.

There are numerous third party web applications you could use to collect data from your users - for example, Google Forms. Or you could use a Filemaker hosting service to host your own.
