GOLive Action Problem

We are attempting to use a Validate and Submit GoLive Action to verify form data before record creation. The Validation works well but we are getting an Error # 4 message (No Valid action in input form) when attempting to submit the form. We suspect the problem maybe in the following code:

You may need to insert "-new" between these tags:

It seems to be striping the "-new" from the submitted data.
