Simple Form... What am I doing wrong?

Hi everyone, I've been trying to make a simple form using Lasso 6 to collect information from website visitors to add them to a mailing list. Here is the site:

I keep getting errors that say it can't add. The two pages I am using to accomplish this are signup.lasso and form_response.lasso.

Also, is this the best way to get the information into the database? Do I have to use two lasso pages to do it, or can this be accomplished with just the signup page?

I know nothing about lasso code, and am working from a tutorial. We have other web enabled databases that work perfectly with Lasso. The contractor who used to do our databases implemented the system, but he's gone now. I'm fairly certain I set up the new database with the same Lasso permissions as the other ones, so I'm pretty sure it's just something I'm doing wrong with the Lasso code.

Any help you guys could provide would be greatly appreciated.
