Simple Order Form for DVD'S

I have a small video production company. I need a simple file maker solution to Keep track of Oders from parents ordering DVD'S of the events will cover., dance recitals, cheerleading, school plays, etc. Just a form to put name and address and the event ordered. Then print out out mailing labels and a report of total ordered of each different event. Thank You
