I'm looking for a solution for a soon-to-be self-employed therapist. Initially a standalone database would be needed, but as FM has web capabilities this would be useful to add at some point in the future.

Anyway... they need to capture client details (name, address etc), medical conditions / treatment information (type and frequency etc) and payment based on each treatments.

An important factor would be to keep a history of treatments / progress etc. somehow listed with each ecord

If anyone knows of an existing solution I'd be happy to hear of it and maybe I can with your help customise it, as I have a little knowledge of FM form a decade ago. They will consider paying if someone here is able to custom write a sultuion.

Perhaps you'd like to explicitly say what version of FileMaker this needs to run on? You've got FileMaker 4 in your profile. That's awfully old.

I actually have version FMP 8.5 now.
