Multiple Signed In Instances for Some Users

Today I found some clients has signed in multiple times to the server. Which caused a major problem like other users/client unable to access the file. So I needed to restarted the server. Then the problem fixed. 

 

I guess due to multiple signed in for some clients, this problem occurred. So can anyone please help me, why multiple signed in for some users happens? And is it related to server version? My current server version is 12.0.4.405
