could i set it up so that i could have a layout with different fields and then send that layout out as a mail message?

like if i have a layout that has, First name, last name, and ss# and the user fills that in, and then they can click on a button and it would send the info through the mail to some one else. and the mail message would look like-

First name: james

Last name: Smith

ss#: 123-45-6789
