newbie database setup - will reward!

Ok Guys, Hello

I just downloaded the trial of filemaker 7 pro for windows. I need a very basic database setup, and one of the 'premade' databases doesn't really work for me.

I would be willing to pay $7.50 by paypal if someone would like to help me setup a basic database with an appropriate form view. I know several people offer free help in these forums, but I think you should be compensated. It is only fair!

The database would be similar to the existing 'family medical records' database, but with several alterations for different fields, and I need to add a few containers. I bet someone could do this in 5-10 minutes, but I need it done fast to see if FM7 is right for me and my group prior to purchasing!

Here is the deal: I work for a hospital and I need to make a database of the patients for research purposes. The fields will be:

Patient Last Name(text)

Patient first name (text)

Hospital number (number)

Diagonsis (text)

Surgery (text)

Phone number (phone number)

Address (text)

Referring MD #1 (text)

Referring MD #2 (text)

Past Medical History (text)

Past Surgical History (text)

Date of Birth (date)

Date of Surgery (date)

Pathology (text)

Preop labs (text)

Preop comments (text)

OR time (number)

Operative Comments (text)

Length of Stay (number)

Complications (number 0= no, 1= yes)

Complications desription (text)

Hospitalization comments (text)

Relevant Hospital Labs (text)

Follow up #1 (text)

Follow up #2 (text)

Follow up #3 (text)

Follw up #4 (text)

Relevant follow-up labs (text)

Image #1 (container for jpegs)

Image #2 (container for jpeg)

Image #3 (container)

Extra Container (container)

I need a form view similar to family medical records - with three tabs

On the main screen will be the fields: everything in my list from name through pathology

On the first tab: (called preop info) - everything in my list from preop labs to hospitalization labs

On the second tab (labelled followup) - from follow#1 to relevant followup labs

On the third tab image #1 to extra container.

Wouldn't it make mare sense for you to attempt this yourself, so you can see how easy or difficult it is?
